Dear Dr. Dixon,

Is there a simple fitness test I can do at home, involving e.g. step-ups where by taking pulse rate changes at various times I can calculate a general fitness level?

There is no one simple fitness test. However there are several of them outlined in a wonderful book produced in 1992 by the American College of Sports Medicine. It is called "ACSM Fitness Book". [ISBN 0-88011-460-6] If your bookstore doesn't have it you can call Leisure Press at Human Kinentic Publishers 800-747-4457. About $12.

The book describes in detail assessing four components of your present fitness state: Cardiorespiratory by the Rockwell one-mile walking test (this one, I think, answers your question best); Muscular strength and endurance by the push-up test; Flexibility by a modified sit-and-reach test; and Body composition by telling you how to measure your Body Mass Index and what it means.
